These Terms of Service ("Terms") govern your access to and use of the website, intake forms, scheduling tools, and services provided by Your Path to Care LLC ("Your Path to Care," "we," "us," or "our"). By using our website or engaging our services, you agree to these Terms.

Nature of Our Services

Your Path to Care provides non-clinical patient advocacy, healthcare navigation, education, and administrative support. We do not provide:

  • Medical advice, diagnosis, or treatment
  • Legal advice or legal representation
  • Insurance brokerage or sales of insurance products
  • Guarantees of coverage, authorization, or appeal outcomes

Final decisions regarding coverage and approvals rest with insurers, government agencies, providers, and other third parties.

Discovery Calls & Scheduling

Complimentary 15-minute discovery calls are scheduled through Calendly. By booking a call, you agree to Calendly's own terms and privacy notice in addition to ours. Information you submit through Calendly (such as your name, email, and answers to scheduling questions) is shared with us so we can prepare for and conduct your appointment.

Discovery calls are informational only and do not create a client relationship. A client relationship is formed only when you sign a service agreement with us.

Intake Forms & Documentation

When you submit an intake form or send documents to us, you represent that the information is accurate to the best of your knowledge and that you are authorized to share it. We use the information solely to evaluate fit and to provide the services you request. Please do not submit sensitive information you do not want us to receive.

Fees & Payment

Fees for paid services are described on our Services page or in your individual service agreement. Payment is due as described at booking or in your agreement. Payments are processed by a third-party payment processor.

Refund Policy

  • Discovery calls are free; no refund is required.
  • Denial Decoded™: Because review work begins immediately upon receipt of documents, these services are non-refundable once review has started.
  • Care Access Intensive™ and similar engagements: Due to the time-intensive nature of records review, research, and strategic analysis, all fees become non-refundable once review of records, denial documentation, or case materials has begun. If you cancel before review begins, a refund may be issued less any administrative processing fees.

Client Responsibilities

  • Provide accurate, complete, and timely information
  • Make your own medical, legal, and financial decisions
  • Submit appeals, forms, or communications to insurers or providers (unless we have agreed in writing to do so on your behalf)
  • Comply with deadlines set by insurers, providers, and government agencies

No Professional Relationship

Use of our website, intake forms, or scheduling tools does not create a clinical, legal, or fiduciary relationship. A professional engagement with Your Path to Care begins only upon execution of a written service agreement.

Confidentiality

We treat client information as confidential and only share it with your written authorization or as required by law. See our Privacy Policy for additional detail.

Disclaimer of Warranties

The website and services are provided "as is" and "as available," without warranties of any kind, whether express or implied. We do not warrant that the services will meet your expectations or that any specific outcome will be achieved.

Limitation of Liability

To the fullest extent permitted by law, Your Path to Care LLC and its owners, employees, and contractors will not be liable for any indirect, incidental, special, consequential, or punitive damages, or any loss of profits or revenues, arising from your use of the website or services. Our total liability for any claim arising out of or relating to the services will not exceed the fees you paid us for the specific service that gave rise to the claim.

Third-Party Services

Our website and services may link to or rely on third-party tools (such as Calendly, payment processors, and email providers). We are not responsible for the content, policies, or practices of those third parties.

Governing Law

These Terms are governed by the laws of the State of Oregon, without regard to its conflict-of-laws principles. Any dispute arising out of or relating to these Terms or our services will be resolved in the state or federal courts located in Oregon.

Changes to These Terms

We may update these Terms from time to time. The "Last updated" date above reflects the most recent change. Continued use of our website or services after a change means you accept the updated Terms.
